TSPEAR, also known as C21orf9, is a 669 amino acid secreted protein. Expressed as two isoforms produced by alternative splicing, TSPEAR contains one Thrombospondin N-terminal domain and seven EAR (epilepsy-associated repeat) domains. EAR domains are found in several proteins, including TSPEAR, encoded by genes that map within chromosome regions associated with seizure disorders. It is thought that the EAR domain plays a role in the pathogenesis of epilepsy by either binding to an unknown epileptic ligand or interfering with axon synaptogenesis.

The synuclein family members, including Alpha-synuclein (also designated NACP for non-Beta-Amyloid component) and Beta-synuclein, are predominantly expressed in the brain and are speculated to be involved in synaptic regulation and neuronal plasticity. Alpha-synuclein is localized to neuronal cell bodies and synapses. Alpha-synuclein was first identified as a component of Alzheimer’s disease amyloid plaques. Abnormal platelet function in Alzheimer’s disease has been demonstrated. During megakaryocytic differentiation Alpha-synuclein has been found to be upregulated, while Beta-synuclein is downregulated, indicating that coordinate expression of synucleins may be important during hematopoetic cell differentiation. A mutant form of Alpha-synuclein has been found in patients with early onset Parkinson’s disease.

NHE-3 are integral membrane proteins that are expressed in most mammalian tissues, where they regulate intracellular pH and cell volume. NHEs mediate the transport of hydrogen (H+) ions out of cells in exchange for extracellular sodium (Na+) ions. While NHE-1 is ubiquitously expressed, the NHE isoforms 2-8 have distinct tissue- and cell type-dependent expression and inhibitory characteristics. NHE-3 localizes to the apical membrane of renal proximal tubules where it is responsible for most of the sodium transport and fluid reabsorption. NHE-3 translocates to internal pools where it mediates natriuresis when blood pressure increases abruptly. NHE-3 is also expressed in the stomach and functions to protect the mucosa by secreting protons that diffuse into the mucous cells.

RBM26 also contains one C3H1-type zinc finger and two RRM (RNA recognition motif) domains. The gene encoding RBM26 maps to human chromosome 13q31.1.
