Interleukin-21 (IL-21) is a key factor in the transition between innate and adaptive immune responses secreted by activated T cells. The IL-21 receptor (IL-21R) is expressed in lymphoid tissue, in particular by NK, B, T and dendritic cells, macrophages and endothelial cells. Recent evidence suggests that IL-21 plays a supportive role in the proliferation of T and B cells and influences the cytolytic activity of natural killer cells. IL-21 has been shown to up-regulate genes associated with innate immunity and to inhibit the differentiation of naive T helper cells. IL-21 specifically inhibits IFN-gamma production from developing TH1 cells and is preferentially expressed by TH2 cells. Furthermore IL-21 has been identified as a growth and survival factor for human myeloma cells. IL-21/IL-21R interactions have a unique role in sequentially activating both innate and adaptive immune responses against poorly immunogenic tumours, leading to tumour rejection that is perforin dependent but IFN-gamma independent.

Interleukin 23 receptor (IL23R) is a type I cytokine receptor for IL23. IL23 receptor complex is comprised of two subunits, the IL12R beta 1 subunit, which is shared with several cytokines, and a subunit that is unique to IL-23. Interleukin-23 receptor (IL-23R), is a member of the type I cytokine receptor family, type 2 subfamily. It is expressed by monocytes, Th1, Th0, NK and dendritic cells. Isoform 1 is specifically expressed in NK cells. IL-23 functions in innate and adaptive immunity and may participate in acute response to infection in peripheral tissues. IL-23 may be responsible for autoimmune inflammatory diseases and be important for tumourigenesis. IL-23R associates with IL-12RB1 to form the interleukin-23 receptor. It binds IL-23 and mediates T cells, NK cells and possibly certain macrophage/myeloid cells stimulation probably through activation of the Jak-Stat signalling cascade. Genetic variations in IL-23R are associated with inflammatory bowel disease type 17 (IBD17). IBD17 is a chronic, relapsing inflammation of the gastrointestinal tract with a complex etiology. Genetic variations in IL-23R also can cause susceptibility to psoriasis type 7.
